When you go on a hike, walk, or birding trip, there are two forms you need: the OLLI Outdoor Liability Waiver and the Emergency Contact Information Form.

Chico State requires us to get a new form from you each term. We will need this form on file in the OLLI office before you go on your hike.
There are a number of ways you can return this to us:
- Email us at olli@csuchico.edu to let us know you would like to receive an Adobe Sign link to sign the form.
- Email a scan or photo of your signed liability form to olli@csuchico.edu.
- Print out your form and mail it to us at 400 W. First Street, Chico, CA 95929-0792.
- Visit our office to fill out a form. We are located in Aymer J. Hamilton, Room 118B on the Chico State campus (pdf map), and we are open Monday-Thursday, 9am-2pm.
Emergency Contact Information
You will need to bring a copy of this with you to each outdoor class you take and turn it in to the class’s paperwork assistant. If your class meets more than once, you only need to bring the form to the first class.
You will need to take a separate copy of this to each outdoor class. We want your instructor to have the most up-to-date information on the day of your hike, and Chico State requires OLLI to keep these on file for each class.
