REGISTRATION

Includes book and admission for you and a partner thanks to the Grays Harbor Family Support Program. Apple Health accepted for eligible participants.

HOW TO REGISTER WITH APPLE HEALTH

1. Complete the Apple Health Billing Request and Consent for Services form online. You will need your ProviderOne ID number or your Social Security number. The form takes a few seconds to load. You do not need to download an app. If you are on your phone, when you click the link above, a SignNow page will open. On that page, find the link that says "Sign With Mobile Browser" near the bottom of the page to complete the form without downloading an app.
2. Return to this page and add the course to your cart.
3. Create an account and checkout. Make sure your email address and phone number are accurate so that we can contact you to complete your registration.
4. When you check out, select "Please bill WA Apple Health" from the payment option dropdown.
5. Complete the registration process. Your registration status will be "Registered, not paid."

After we receive your completed form and registration, we will verify your enrollment in Apple Health and finalize your registration in the class. Your registration status will be changed to "Registered" and we will let you know by email and phone. If we have any questions or problems, we'll reach out to you. You will receive registration information by email, and you can check your course registration status in your Account at any time.
